Transitioning to home from a facility can be a critical time. Mistakes are often made that may slow recovery. Our Agency understands that the health care system can be complicated to navigate, especially at such a stressful time. It can be difficult to remember and keep track of all of the instructions, appointments, and medication changes when you’re being discharged home for the hospital or rehab facility.

How the Program Works:
First, a Nurse Liaison from Home Care Solutions will meet with you and your family to assist with this transition. With your permission, we can either meet at the hospital or upon arriving home. Your Nurse Liaison’s job is to make sure that you have everything in place, so that you can continue to get well once you arrive at home. For example, your Nurse will make sure that your medicines have been ordered, your follow up appointments have been made, and any ordered medical equipment has been delivered.

Our caregivers will then work with you and your family to make sure that you fully understand the ongoing needs for recovery at home including:

  • Knowledge of all follow up appointments

  • Changes to your medication regimen

  • Dietary recommendations/restrictions

  • Safety precautions to be taken once home

  • Any changes that could mean you or your loved one may be declining

Once you’re home, a Home Care Solutions Caregiver can provide assistance with:

  • Meeting you or your loved one at the facility and providing safe transportation home

  • Picking up prescriptions, groceries, and/or supplies

  • Preparation of meals and encouraging proper hydration and nutrition

  • Monitoring compliance to make sure prescribed medications are being taken appropriately

  • Encouraging continuation of strength building exercises at home per Physical Therapist and Transportation to follow up appointments
